Booking your appointment
To book an appointment, please call us on (02) 4313 2004.

We will review your referral and confirm an appointment date and time with you within 48 hours (may be longer over weekends or public holidays).
​
We may ask you to have further blood tests, urine tests or scans done prior to your appointment.
​
All appointments will be reconfirmed within the week prior to your scheduled date. If we cannot get in contact with you about your appointment or you are unable to confirm your appointment, your appointment time will be given to another patient.
​
For urgent appointments, we ask your referring doctor speak to Dr Latif to organise your review without delay, or organise hospital admission if required.
What to bring to your appointment
Please bring with you:

Current GP referral (valid for 12 months from date of letter) or other specialist referral (valid for 3 months from date of letter)

Medicare card

Pension or DVA card
-
Private health fund details
-
Recent scan results
-
Blood and urine test results​
Cancellations & no show policy
If you need to cancel your appointment we require at least 48 hours notice so we can offer your appointment to another patient.​
​

Failure to attend your appointment may lead to lengthy delays in rebooking due to the busy nature of the practice.
